Congratulations! You have an opportunity to purchase equipment from Johnson and Johnson, one of the largest healthcare companies. 1. You must be the end user of the equipment being purchased and have intentions to install it for your own use.

3. You will need to submit an insurance certificate which reflects certain insurance information outlined below:

1. Insurance Certificate in name of buyer; buyer is an end user
2. General/product liability meets limits required for assets being purchased
3. Worker's compensation liability meets required limit
4. Waiver of subrogation applies in favor of JJSI, Alios BioPharma - South San Francisco, and its affiliates with respects to General Liability, Product Liability and Workers Compensation where agreed to by contract prior to date of loss
5. Policy is effective at time of transaction and should be valid for at least one year
6. Policy includes language that J&J will be notified upon cancellation or expiration
